Supplemental health plans are available to all full-time team members and their eligible dependents.
If you or a covered dependent is injured (off the job) or needs covered medical tests, services, or treatment—including ER visits or life/dismemberment losses—you’ll get a lump-sum payment.
The amount depends on the severity of the injury. These payments are in addition to any other coverage you have. To submit a claim, complete the Accident Insurance Claim Form.
If you're diagnosed with a serious medical condition like cancer, heart attack, stroke, severe burns, paralysis, or advanced Alzheimer's, you'll get cash benefits. The payment is yours to use however you need, and it's in addition to any other coverage you have.
You can choose coverage of up to $30,000 for yourself. The coverage for your spouse/domestic partner can't be more than 50% of your coverage amount, and coverage for children (up to age 26) cannot be more than 25%. To submit a claim, complete the Critical Illness Insurance Claim Form.
